Billing

Fees for therapy:

  • Intake session (for individual or couples therapy): $160 for 60 minutes

  • Individual therapy: $140 for 50-55 minutes

  • Couples therapy: $150 for 50-55 minutes

  • Missed appointment: Full session fee (see above). Insurance generally does not reimburse for a missed appointment

Payment-in-full is due at the time of service. I accept payment by cash, check, or credit card. Please make your check payable to "Sparrow's Spring Clinical Services."

I generally take payment and confirm further appointments at the end of the session. I require a credit card to be on file which will be stored encrypted and, therefore, securely. 

Insurance

I am in-network for Blue Cross Blue Shield PPO. The name of the practice in which I work is "Sparrow's Spring Clinical Services." 

If you wish to use your benefits from another insurance company, I will be happy to provide you with a detailed receipt for your payment so that you can submit your claims to your insurance provider.

What is and is not covered by insurance can be confusing. If you will be using your health insurance to pay my fees, I suggest that you call your insurance company (you will likely be asked for your insurance details from your card), and ask the following questions, as they are appropriate to your situation:

  • Is mental health care covered by my plan?

  • Is individual therapy covered by my plan? (CPT code 90837)

  • Is couples therapy covered by my plan? (CPT code 90847)

  • Is the provider type “Licensed Marriage and Family Therapist” (LMFT) covered by my plan?

  • How many sessions are covered per year?

  • Is "Sparrow's Spring Clinical Services" an in-network provider for my plan? If the answer is "yes", we can bill Blue Cross Blue Shield of Illinois directly and you will only be responsible for your co-pay at the time of your appointments.

  • If "Sparrow's Spring Clinical Services" is an out-of-network provider, how much of the fee will I be reimbursed for each visit?

  • What is my deductible? (the amount you will have to pay out of pocket {usually annually} before insurance covers your medical expenses)

  • How much of my deductible has already been met?

  • Do I have a co-pay for each visit, and if so, what is that fee? You will pay that amount directly to me at the time of each visit.

I will be happy to provide you with any documentation that you need to submit to your insurance company. Just so you know, in order to bill your insurance company, I have to submit a psychiatric diagnosis to them. Some people are uncomfortable with this and choose to pay out of pocket to maintain complete confidentiality. I am happy to discuss this further to help you consider your options.
